What is Homocysteine?

Homocysteine is a sulphur-containing amino acid that forms naturally as the body breaks down the dietary amino acid methionine. It does not come from food directly; instead, it accumulates as a metabolic intermediate and is normally converted onwards with the help of B vitamins — particularly folate, B12, and B6. Measuring it in blood gives a useful snapshot of how well these B-vitamin-dependent pathways are functioning, and elevated levels have been associated with an increased risk of cardiovascular disease, cognitive decline, and bone fragility.

What a high Homocysteine means

A raised homocysteine result suggests that the body's methylation cycle may not be running efficiently — most commonly because of a nutritional shortfall or an underlying condition affecting B-vitamin metabolism. It is a prompt to investigate further rather than a diagnosis in itself.

  • Low B12, folate, or B6. These three B vitamins are the principal cofactors needed to clear homocysteine from the bloodstream. Dietary insufficiency — common in people eating little meat, fish, dairy, or eggs — or poor absorption (for example, due to pernicious anaemia or long-term use of proton pump inhibitors) can allow levels to rise.
  • Genetic variants (MTHFR). Variants in the MTHFR gene, particularly C677T, reduce the efficiency of folate metabolism and are relatively common in the UK population. They can push homocysteine higher, especially when dietary folate intake is already low. A GP can arrange testing if this is suspected.
  • Kidney disease or hypothyroidism. The kidneys play a role in homocysteine clearance, so impaired kidney function can cause levels to accumulate even when B-vitamin status is adequate. Underactive thyroid (hypothyroidism) is another recognised cause, and thyroid function is often checked alongside this result.

Certain medicines — including methotrexate, some anticonvulsants, and long-term metformin — can also raise homocysteine by interfering with B-vitamin metabolism; always mention your current medications to your GP when discussing this result.

What a low Homocysteine means

What actually helps

Because elevated homocysteine is most often driven by B-vitamin insufficiency, the most evidence-backed approach is ensuring adequate intake of the three key cofactors.

  • Optimise folate and B12 intake. Folate and B12 are the two most influential nutrients for homocysteine clearance. If blood tests confirm a deficiency, your GP may recommend supplementation; for those with dietary gaps, increasing intake through food is a sensible first step.
  • Ensure adequate B6. Vitamin B6 supports a separate pathway (transsulphuration) that also clears homocysteine. Most people in the UK get sufficient B6 from a varied diet, but it is worth checking intake if levels remain raised.
  • Consider methylfolate if MTHFR variant is present. People with the MTHFR C677T variant may convert standard folic acid less efficiently; some clinicians recommend the active form, methylfolate (5-MTHF), in this context. Discuss with your GP before switching supplements.
  • Adequate hydration and kidney health. Since the kidneys help clear homocysteine, supporting overall kidney health — including staying well hydrated and managing blood pressure — is a sensible background measure, particularly for those with raised levels not explained by B-vitamin status.

Dark green leafy vegetables (spinach, kale, broccoli)

Among the richest dietary sources of folate, the key driver of homocysteine remethylation.

Legumes (lentils, chickpeas, beans)

High in folate and plant protein; regular consumption is associated with lower homocysteine in observational studies.

Oily fish and lean meat

Provide B12 and B6 in highly bioavailable forms; B12 is found almost exclusively in animal-source foods, making these important for those who eat them.

Eggs

A practical source of both B12 and choline, which supports methylation pathways related to homocysteine metabolism.

Beetroot and wholegrains
