What is Neutrophil Count?

Neutrophils are the most abundant type of white blood cell and form a critical part of the body's innate immune system, acting as a rapid-response force against bacterial and fungal infections. They are produced continuously in the bone marrow and released into the bloodstream, where they circulate for only a matter of hours before migrating into tissues. Measuring the neutrophil count as part of a full blood count (FBC) gives clinicians a snapshot of immune activity and can help detect infection, inflammation, or problems with bone marrow function.

What a high Neutrophil Count means

A raised neutrophil count (neutrophilia) is most commonly a sign that the body is mounting an immune response, but it is a signal to look closer rather than a diagnosis in itself — context, symptoms, and other blood markers all matter.

  • Infection. Bacterial infections are the most frequent cause of a raised neutrophil count, as the bone marrow rapidly releases more neutrophils to fight pathogens. Viral infections can also cause a transient rise, though they more typically cause a relative increase in lymphocytes.
  • Physiological stress & inflammation. Physical or psychological stress, strenuous exercise, surgery, trauma, and systemic inflammatory conditions such as rheumatoid arthritis can all trigger neutrophilia. Smoking is also a well-recognised cause of a persistently elevated count.
  • Medications & other causes. Corticosteroids (widely prescribed in the UK for conditions such as asthma or inflammatory bowel disease) are a common drug cause of raised neutrophils. Rarely, a persistently very high count may prompt investigation for a bone marrow or blood disorder.

A single mildly raised result in someone who is unwell or has recently exercised or smoked is usually not concerning; repeat testing once any acute trigger has resolved is the standard approach.

What a low Neutrophil Count means

A low neutrophil count (neutropenia) can reduce the body's ability to fight certain infections and is generally the more clinically significant direction for this marker, warranting prompt GP review if the count falls clearly below the laboratory reference range.

  • Viral infections & post-viral states. Many viral illnesses — including influenza and COVID-19 — can temporarily suppress neutrophil production or increase their destruction, leading to a transient neutropenia that typically resolves as the infection clears.
  • Medications. Several medications, including some antibiotics, anti-thyroid drugs, and chemotherapy agents, can suppress bone marrow output and reduce neutrophil counts. If you have recently started a new medicine, your GP will consider this as a potential cause.

A mild, isolated, and transient low count in an otherwise well person — particularly in people of Black African or Caribbean ancestry, where a benign constitutional neutropenia is common — may not require treatment, but should be discussed with a GP.

Should I be worried?

A single out-of-range neutrophil count, particularly if only mildly abnormal, is rarely cause for immediate alarm — many everyday factors, from a recent cold to a morning jog, can shift the count temporarily. What matters most is the pattern over time, the degree of abnormality, and how it sits alongside your other blood markers and symptoms. If your result is flagged as low, especially if you are also experiencing frequent infections or feeling persistently unwell, it is worth speaking to your GP promptly. A high count in the context of an obvious infection or clear stressor is usually self-limiting, but a persistently elevated or markedly abnormal result — in either direction — should be followed up, as your GP may want to repeat the FBC, review your medications, and check other markers such as your CRP, differential white cell count, or haemoglobin.

What actually helps

While neutrophil counts are largely determined by immune status, bone marrow health, and underlying conditions, certain lifestyle habits support healthy immune cell production and regulation.

  • Avoid smoking. Smoking is one of the most well-established causes of chronically elevated neutrophil counts; stopping smoking typically leads to a gradual normalisation over weeks to months, as well as broader cardiovascular and respiratory benefits.
  • Manage infections promptly. Ensuring infections are identified and treated appropriately — and keeping vaccinations up to date in line with the NHS vaccination schedule — reduces the frequency of acute neutrophil-altering immune challenges.
  • Moderate-intensity exercise. Regular, moderate exercise supports healthy immune function and is not associated with the neutrophil-raising effects of acute intense exertion. Aim for the UK Chief Medical Officers' guideline of at least 150 minutes of moderate activity per week.
  • Limit alcohol. Heavy alcohol consumption can suppress bone marrow function and impair neutrophil production; keeping within UK low-risk drinking guidelines (no more than 14 units per week, spread across several days) supports immune resilience.

There is good supporting evidence that adequate intake of the following nutrients and foods helps maintain healthy white blood cell production and immune function.

Lean meat, fish & eggs

Rich sources of protein, zinc, and vitamin B12, all of which are essential for normal bone marrow function and neutrophil maturation.

Leafy green vegetables (e.g. spinach, kale)

Provide folate, which is required for DNA synthesis in rapidly dividing bone marrow cells; folate deficiency is a recognised cause of reduced white cell production.

Fortified cereals & dairy

Good dietary sources of vitamin B12 and vitamin D, both of which support normal immune cell development; vitamin B12 deficiency is a well-known cause of cytopenias.

Nuts and seeds (e.g. pumpkin seeds, cashews)

Provide zinc and selenium, trace minerals with established roles in supporting neutrophil function and production.

Oily fish (e.g. salmon, mackerel, sardines)

A source of omega-3 fatty acids, which have anti-inflammatory effects that may help prevent chronic low-grade immune activation associated with persistently elevated neutrophil counts.
