What is Platelets?
Platelets are tiny, disc-shaped cell fragments produced in the bone marrow from large cells called megakaryocytes. When a blood vessel is damaged, platelets rush to the site, clump together, and help form a clot to stop bleeding — a process called primary haemostasis. Measuring the platelet count in a full blood count (FBC) gives clinicians a quick snapshot of your clotting capacity and can flag a range of conditions affecting the bone marrow, immune system, or circulation.
What a high Platelets means
A platelet count above the upper reference limit — known as thrombocytosis — is a signal that something may be driving increased platelet production or release, and warrants further investigation rather than alarm. It is rarely an isolated finding and is usually considered alongside your symptoms and other blood results.
- Reactive thrombocytosis. The most common cause in primary care: the bone marrow produces more platelets in response to acute or chronic inflammation, iron-deficiency anaemia, or tissue injury. Treating the underlying condition usually brings the count back into range.
- Iron-deficiency anaemia. Low iron stores are a well-recognised trigger for a raised platelet count. Your GP may check ferritin and a full iron panel alongside the FBC if this is suspected.
- Bone-marrow or haematological conditions. A persistently high count — particularly when other blood parameters are also abnormal — can occasionally point to a primary bone-marrow disorder such as essential thrombocythaemia, which a haematologist would investigate with further tests including a JAK2 gene mutation screen.
A mildly elevated platelet count following recent infection, surgery, or during pregnancy is common and often resolves on its own; a single reading needs context before any conclusions are drawn.
What a low Platelets means
A platelet count below the lower reference limit — thrombocytopenia — can increase the risk of bruising and bleeding, and the significance depends on how low the count is and whether it is falling. Even mildly low counts are worth discussing with a GP to understand the cause.
- Viral infections and illness. Many viral infections — including glandular fever and, notably, COVID-19 — can temporarily suppress platelet production or increase platelet destruction. Counts usually recover once the infection resolves.
- Medications. A range of commonly prescribed medicines, including certain antibiotics, diuretics, and anticonvulsants, can reduce platelet counts as a side effect. Your GP or pharmacist can review whether any current medication could be a factor.
- Autoimmune causes. In immune thrombocytopenic purpura (ITP), the immune system mistakenly destroys platelets. It can occur at any age and is one of the more common causes of an isolated low platelet count in otherwise healthy people; a haematology referral may be needed.
Alcohol excess is also a recognised cause of a low platelet count, both through direct bone-marrow suppression and via liver disease affecting the spleen.
Should I be worried?
A single out-of-range platelet count is often a prompt for a repeat test rather than a definitive finding — transient changes are very common and can reflect nothing more than a recent illness or a busy immune system. That said, a result that is significantly outside the reference range, is accompanied by symptoms such as unexplained bruising, bleeding gums, petechiae (small red dots on the skin), or persistent fatigue, or that remains abnormal on a repeat test, should be discussed with your GP promptly. They may repeat the FBC, check inflammatory markers such as CRP, assess iron stores, or refer to haematology depending on the clinical picture. What actually helps
Because an abnormal platelet count is almost always driven by an underlying condition, the most important lever is identifying and addressing that cause with your GP — but certain lifestyle factors can support healthy bone-marrow function and reduce unnecessary inflammation.
- Address iron deficiency. If low iron is contributing to a raised platelet count, working with your GP to restore iron stores — through diet or supplementation — often normalises the count. Do not supplement iron without a confirmed deficiency.
- Limit alcohol. Reducing alcohol intake supports healthy bone-marrow function and can improve a low platelet count related to alcohol use; the NHS recommends keeping within 14 units per week and having several alcohol-free days.
- Manage chronic inflammation. Chronic low-grade inflammation is a key driver of reactive thrombocytosis. Regular moderate exercise, a balanced diet, and good sleep hygiene all have good evidence for reducing systemic inflammatory markers such as CRP.
- Follow up medications. If a medicine is suspected to be affecting your platelet count, speak with your GP before stopping anything — they can advise on alternatives or monitoring. Never discontinue prescribed medication without guidance.
