What is Albumin?

Albumin is the most abundant protein in the bloodstream, produced continuously by the liver. It plays several vital roles: carrying hormones, medications, and fatty acids around the body, maintaining fluid balance between blood vessels and tissues, and acting as a buffer in the blood. Because the liver must be working well and the body must have sufficient protein to make albumin, blood levels reflect both liver synthetic function and overall nutritional status, making it a useful and widely used clinical marker.

What a high Albumin means

A result above the upper end of the reference range is most commonly a sign of dehydration rather than a problem with the liver itself. It is worth noting alongside other markers, but it rarely signals a serious underlying condition on its own.

  • Dehydration. When fluid levels in the blood are low, proteins like albumin become relatively concentrated, pushing the measured value upward. Ensuring adequate fluid intake and re-testing usually resolves this finding.
  • Prolonged tourniquet use. If the tourniquet was left on the arm for too long during a blood draw, fluid can shift out of the vein and artificially raise albumin. This is a laboratory or technique artefact rather than a true physiological change.
  • High protein intake around the test. While albumin does not respond acutely to a single meal, consistently very high protein intake over time can sit at the upper end of normal; this is generally not a concern.

A mildly elevated albumin in isolation is rarely clinically significant. If dehydration is suspected, your GP may simply advise rehydration and repeat testing.

What a low Albumin means

A low albumin result is worth taking seriously, as it can reflect reduced liver synthetic function, significant nutritional deficiency, or protein loss through the kidneys, and warrants further evaluation by a GP.

  • Chronic liver disease. Conditions such as cirrhosis impair the liver's ability to produce albumin, so a falling level over time is one of the markers clinicians use to assess how well the liver is coping. It is usually interpreted alongside other liver function tests.
  • Malnutrition or very low protein intake. The liver needs a steady supply of amino acids from dietary protein to synthesise albumin. Prolonged inadequate intake, eating disorders, or malabsorption conditions such as coeliac disease can all reduce levels.
  • Kidney disease (nephrotic syndrome). In certain kidney conditions, the filtering system becomes leaky and albumin is lost in the urine. This can cause a significant drop in blood albumin and is often accompanied by swelling, particularly around the ankles.
  • Chronic inflammation. In long-standing inflammatory conditions — including autoimmune diseases, infections, and some cancers — the liver down-regulates albumin production as part of the acute-phase response. Low albumin in this context is sometimes described as an 'inflammatory low' rather than a nutritional one.

Low albumin is a non-specific finding, meaning it can have several causes. A single low result should always be interpreted in the context of symptoms, other blood tests, and clinical history rather than in isolation.

Should I be worried?

A single albumin reading outside the reference range does not in itself constitute a diagnosis. A mildly low result, particularly if you have been unwell recently or have reduced your food intake, may normalise on its own once you recover, and your GP may simply recommend a repeat test in a few weeks. However, a persistently low albumin, or one that sits alongside abnormal results for other liver markers (such as ALT, bilirubin, or ALP), kidney function tests, or signs of swelling and fatigue, is worth discussing with your GP promptly. They can review the full picture, ask about symptoms, and decide whether further investigation — such as urine protein testing, imaging, or specialist referral — is needed. Equally, if your result is mildly high and you have been drinking less fluid than usual, focus on good hydration and recheck before drawing conclusions.

What actually helps

Because albumin reflects both liver health and nutritional status, the most evidence-backed levers relate to dietary protein adequacy and reducing chronic inflammation.

  • Adequate protein intake. The liver requires a consistent supply of amino acids to synthesise albumin. UK dietary reference values suggest most adults need around 0.75 g of protein per kilogram of body weight per day, with higher amounts sometimes appropriate in older adults or during recovery from illness.
  • Address chronic inflammation. Identifying and managing underlying inflammatory conditions — whether through lifestyle, medication, or both — can help normalise albumin levels that have been suppressed by the acute-phase response. Speak to your GP if you suspect an undiagnosed inflammatory condition.
  • Limit alcohol intake. Alcohol is a direct hepatotoxin and a leading cause of chronic liver disease in the UK. Keeping within the NHS recommended limit of 14 units per week, spread across at least three days, helps protect the liver's ability to synthesise albumin over the long term.
  • Maintain a balanced overall diet. Micronutrient deficiencies — particularly zinc and B vitamins — can impair liver protein synthesis. A varied, whole-food diet supports both the liver and the immune system, helping to sustain healthy albumin levels.
