What is Bilirubin?
Bilirubin is a yellow pigment produced when the body breaks down haemoglobin from old or damaged red blood cells. The liver captures bilirubin from the bloodstream, processes it, and excretes it in bile, which eventually leaves the body via the gut. Measuring bilirubin in the blood gives a useful snapshot of how well this recycling and clearance pathway is working, reflecting both red blood cell turnover and liver and bile duct function.
What a high Bilirubin means
A raised bilirubin level suggests that bilirubin is either being produced faster than usual or is not being cleared efficiently — it is a prompt to look more closely at which part of the pathway is involved, not a diagnosis in itself.
- Gilbert's syndrome. This is a common, benign inherited variation in which the liver processes bilirubin slightly more slowly than usual. It affects roughly 1 in 20 people in the UK and typically causes mild, fluctuating bilirubin rises, particularly during fasting, illness, or periods of stress — it requires no treatment.
- Haemolysis. If red blood cells are breaking down more rapidly than normal — due to causes ranging from certain inherited anaemias to medication reactions — more bilirubin enters the bloodstream than the liver can comfortably clear, pushing levels upward.
- Liver or bile duct dysfunction. Conditions affecting the liver (such as hepatitis or cirrhosis) or obstruction of the bile ducts (for example by gallstones or, less commonly, a tumour) can impair bilirubin excretion, leading to a build-up in the blood and sometimes visible jaundice.
Bilirubin can rise transiently after prolonged fasting, strenuous exercise, or during an acute illness, particularly in people with Gilbert's syndrome, so a mildly elevated result in isolation may warrant a repeat under normal conditions.
What a low Bilirubin means
A low bilirubin level is not considered clinically significant and does not generally require any investigation or action.
Should I be worried?
A single mildly raised bilirubin reading is often benign — Gilbert's syndrome alone accounts for a large proportion of incidentally elevated results — and one result rarely tells the whole story. That said, it is worth discussing any out-of-range value with your GP, especially if it is significantly elevated, if other liver markers (such as ALT, ALP, or GGT) are also raised, or if you have noticed symptoms such as yellowing of the skin or eyes, dark urine, pale stools, or unexplained fatigue. Your GP may repeat the test under standardised conditions, check a full liver panel and full blood count, and consider your personal and family history before deciding whether further investigation is needed.
What actually helps
For most people, bilirubin does not require active lifestyle management, but if your results are mildly raised — particularly in the context of Gilbert's syndrome — a few practical habits can help keep levels stable.
- Eat regular meals. Prolonged fasting is a well-recognised trigger for bilirubin rises in people with Gilbert's syndrome; eating at regular intervals throughout the day helps avoid the spikes that fasting can provoke.
- Stay well hydrated. Adequate fluid intake supports normal liver perfusion and bile flow, helping the liver clear bilirubin efficiently as part of its everyday function.
- Limit alcohol. Alcohol is directly toxic to liver cells and can impair bilirubin processing; keeping within the UK Chief Medical Officers' low-risk guideline of no more than 14 units per week, spread across several days, is a practical minimum target.
- Maintain a healthy body weight. Excess weight, particularly around the abdomen, is associated with metabolic-associated steatotic liver disease (MASLD), which can compromise overall liver function; even modest weight loss of 5–10% body weight can meaningfully improve liver health markers.
Good supporting evidence points to a broadly liver-friendly dietary pattern as the most practical nutritional approach for maintaining healthy bilirubin clearance.
Coffee
Moderate coffee consumption (around 2–3 cups per day) is consistently associated in observational research with lower rates of liver inflammation and fibrosis, likely through antioxidant and anti-inflammatory mechanisms.
Cruciferous vegetables (broccoli, Brussels sprouts, cabbage)
These contain compounds such as glucosinolates that support liver detoxification pathways, including those involved in conjugating and excreting bilirubin.
Oily fish (salmon, mackerel, sardines)
Rich in omega-3 fatty acids, oily fish is associated with reduced hepatic fat and inflammation in multiple well-replicated studies, supporting overall liver function.
Wholegrains and legumes
High-fibre foods support gut transit and help excrete bile acids (which carry processed bilirubin) efficiently, reducing enterohepatic recirculation.
Berries and other antioxidant-rich fruits
Polyphenols found in berries have shown anti-inflammatory effects in liver tissue in clinical studies, complementing the liver's own antioxidant defences.
Unsaturated fats (olive oil, avocado, nuts)
Replacing saturated fat with unsaturated sources is linked to improved liver fat profiles and is a cornerstone of Mediterranean-style eating, which has the strongest overall evidence base for liver health.
